A suggestion (seen done in Parisian bistros): with a cookie cutter (a small glass will do), cut off a circular hole in the slice of bread on the top, put the raw egg in the cavity and toast the croque-madame under the grill/broiler. Top-notch presentation and still delicious as ever.
